Establishing an effective interprofessional team partnership requires awareness of the concept of shared governance, relationship coordination, role definition, and principles of diversity, equity, and inclusion (DEI). Communication frameworks to minimise avoidable harm, respect, and common goal definitions are essential to an effective interprofessional team (Hüner et al., 2023; Spitzer et al., 2023). A literature review showed that there was a significant decrease in hospital readmission rates and a detection of complications in the early stage (Clark et al., 2025; Zhang et al., 2025). The transitional care model ensures patient adherence, increased patient satisfaction, and caregiver engagement. The outcomes of inclusive leadership include psychologically safe environments that ensure team innovation and equity-based outcomes (Yousaf et al., 2022). Findings are congruent with standardized interprofessional discharge bundles based on systems thinking. Patterns of decision making, plan-do-study-act processes, standard communication styles, and equitable outcome measurement develop a collaborative mindset in teams, where innovation is promoted. As interventions are tested, adapted, measured, and supported by policy, innovation is converted into evidence-based practice.
